Creating the Questionnaire
Use the Define Questionnaire page to create the questionnaire which contains the categories, sub-categories, sub-headings, questions and answer layouts.

To create the questionnaire:
1. In the main menu, choose Define Questionnaire.
2. Click the New button to create the questionnaire.
Entry fields display for up to three questionnaires.
3. Enter the questionnaire name and description.
In our example, enter General Appraisal, and General appraisal for all employees.
4. If you previously created a questionnaire containing similar categories and questions, you can copy the categories and questions to the new questionnaire. Select the questionnaire from the Copy From field and click the Save button.
5. When you're done, click the Save button.
